Here’s How it Works:

Health care professionals in hospitals, clinics, emergency rooms etc., administer the Behavioral Health Screen, which a patient can complete on a cell phone, tablet, or home desk top. It identities risk factors that affect health and outcomes to medical care and suggests evidence-based treatments. Then, a behavioral health therapist, medical prescriber, clinic or treatment center is identified through a Referral Management Center or a web-based listing site, and they will receive the Behavioral Health Screen along with medical records.


Patients are actively engaged in their care by receiving user friendly versions of the screen and having the ability to choose a provider themselves online. A medical facility may route the referrals to internal staff or expand the range of mental health services throughout the community and state.

Integrated care is further facilitated through secure messaging that includes patient access. Consent forms, intake assessment and notes are completed online to promote interoperability with a variety of medically-based, HIPAA-compliant telehealth and record storage platforms. All of this available on an interoperable patient chart available to both mental health and medical health professionals.

The web of specialized treatment options is available across the state via Telehealth, giving your client highly personalized care. Your company can collaborate with Therapractic to manage this service or utilize your own staff.
